Job summary

HCA Florida Lake City Hospital, part of HCA Healthcare, seeks a Registered Nurse Case Manager PRN to support patients and families through the care journey. You will coordinate services, develop care plans, and work with physicians and the care team to ensure safe transitions.

Responsibilities include guiding patients through orientation, coordinating education, documenting assessments, and promoting compassionate, high-quality care.

Qualifications

• Must be a licensed RN with an Associate Degree.
• 1 year of experience required.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare/insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to support participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs, and goals.
• Complete psychosocial assessments and discharge planning evaluations, document findings, and communicate needs to the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and the interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence in every interaction.
