HCA Florida Bayonet Point Hospital in Hudson, FL is seeking a Part- or PRN-time Registered Nurse - Case Manager to join our multidisciplinary team. This role focuses on coordinating plans of care, managing length of stay, and ensuring safe discharge planning in collaboration with physicians and social services.

The RN Case Manager PRN will assess patient needs, identify-risk individuals, and promote efficient patient throughput while balancing quality care with appropriate resource utilization.

Qualifications

• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ years in case management OR 3+ years in clinical nursing required.

Responsibilities

• Provide case management services for inpatient and observation patients.
• Identify patients at risk during transitions between care settings.
• Assess psychosocial, medical, and discharge needs of patients and families.
• Reassess clinical condition as indicated.
• Coordinate plan of care and discharge with multidisciplinary team and physician.
• Collaborate with Social Services to ensure appropriate post-acute care.
