Qualifications

• APRN or RN licensure required.
• Associate Degree or RN Diploma required.

Responsibilities

• Guiding patients and families through program orientation and discharge planning.
• Coordinating education for patients, families, and caregivers to support treatment planning.
• Developing individualized treatment plans reflecting the patient’s strengths and needs.
• Completing psychosocial assessments and documenting findings for the care team.
• Collaborating with Rehab Program Director and interdisciplinary team for patient-centered care.
• Upholding the values of compassion, respect, and excellence in interactions.
