Job summary

Healthcare Services Group, Inc. is seeking a PRN Registered Dietitian to support clinical nutrition services at a long-term care facility in Wisconsin. This role could develop into a permanent part-time or full-time position based on staffing and performance.

You will assess residents' nutrition needs, create care plans, provide education, and collaborate with interdisciplinary teams while ensuring compliance with CMS and state regulations. Strong communication and organization are essential.

Qualifications

• Registered by the Commission on Dietetic Registration.
• Licensure in good standing in the state of practice.
• Knowledge of state survey process and nutrition regulations (preferred).
• Strong organizational and communication skills.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop and document the plan within the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical findings, labs, weight trends, intake, and comorbidities.
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to improve residents'/patients’ health and well-being.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs.
• All other duties as assigned.
