Job summary

Creative Solutions in Healthcare - East is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. You will support residents to improve mobility, strength, and quality of life under the supervision of a Physical Therapist.

The role involves administering prescribed PT treatments, documenting progress, and collaborating with the interdisciplinary team to adapt plans of care. Excellent communication and a patient‑centered approach are essential.

Qualifications

• Active and valid Texas Physical Therapy Assistant license required.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer medically prescribed physical therapy treatments under supervision to relieve pain and improve function.
• Provide skilled PT services under the supervision of a Physical Therapist as per physician orders.
• Respond to requests for service by relaying information and referrals to the Physical Therapist.
• Implement and modify plans of care under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient response and modify approaches with the interdisciplinary team.
• Provide clinical support and education to patients, families, and caregivers.
• Ensure thorough and timely documentation in line with policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
