Job summary

Creative Solutions in Healthcare - East is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. You will deliver person-centered care to help residents improve mobility and quality of life under supervision of a Physical Therapist.

You must hold an active Texas PTA license, have strong communication and teamwork skills, and prior SNF or long-term care experience is preferred.

Qualifications

• Active and valid Texas PTA license.
• Strong communication and teamwork skills.
• Experience in Skilled Nursing Facility or long-term care preferred.
• Knowledge of physical therapy practices and documentation.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services under the supervision of a Physical Therapist.
• Respond to requests for services and relay information to the Physical Therapist.
• Implement and adjust plans of care under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adjust approaches with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Ensure thorough and timely documentation per policies and regulations.
• Attend required rehab department meetings.
