Job summary

Creative Solutions In HealthCare is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehabilitation team on a PRN basis. You will support residents by delivering prescribed PT, tracking progress, and helping maintain mobility and quality of life.

Under PT supervision, you will work with an interdisciplinary team, document thoroughly, and communicate patient needs. In-house rehab allows flexible scheduling and a stable clinical environment.

Qualifications

• Active Texas PTA license is required.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health documentation system.
• Ability to collaborate with an interdisciplinary team.

Responsibilities

• Administer prescribed PT treatments to relieve pain and improve function.
• Provide PT services under supervision of a Physical Therapist.
• Relay information and referrals to the PT on request.
• Modify plans of care under PT supervision.
• Measure and record motor function, strength and performance.
