Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in East Moline, IL. The role supports the Physical Therapist to deliver resident-centered therapy services.

Responsibilities include screenings, treatment sessions (individual and group), documentation in NetHealth EMR to meet CMS standards, and collaboration with the care team to achieve rehab goals.

Qualifications

• Requires PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluating residents and developing goals and treatment plans.
• Conduct both individual and group treatment as outlined in the treatment plan.
• Monitor residents’ progress and modify plan as indicated.
• Document therapy activities in NetHealth EMR per CMS expectations.
