Job summary

Continuum Therapy Partners in Wisconsin is hiring a Physical Therapist to join our PRN daytime roster in Sturgeon Bay. You’ll deliver high-quality care with the same standards as our full-time team and set flexible scheduling to fit your life.

The role requires a Doctor of Physical Therapy, an active PT license, and passing NPTE, with 1 year of experience preferred. You’ll educate patients, monitor progress, and collaborate with a multidisciplinary team.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
