Job summary

Continuum Therapy Partners in Kewaunee, WI is seeking a licensed Physical Therapist to join our PRN team. You’ll provide high-quality care with flexible daytime shifts and collaborate with our leadership team to maintain consistent standards.

Requirements include a Doctor of Physical Therapy (DPT), an active state license with NPTE, and about 1 year of experience preferred. You’ll educate patients and families on home programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
