Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to provide in-house rehabilitation for Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona and Missouri. This role offers an hourly rate of $45 with consistent hours and the opportunity to contribute to patient recovery and quality of life.

Applicants should hold a valid LPTA license. The position is open to therapists at all experience levels and requires delivering evidence-based, skilled therapy under

• Licensed Physical Therapy Assistant (LPTA) with valid license.
• Open to therapists at all experience levels.
• Experience applying evidence-based practice in rehab.

Responsabilidades

• Utilizes evidence-based practice to support clinical interventions.
• Provides skilled therapy services to patients with physical functioning disorders under supervision.
• Assesses and documents patient progress and communicates with supervising PT to adjust treatments.
• Provides consultation to patients and families regarding physical disorders.
• Contributes to discharge planning and summaries of services.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
