Job summary

Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to join our clinical team in Texas. The role emphasizes evidence-based interventions and compassionate patient care within in-house therapy programs at Skilled Nursing facilities.

The ideal candidate holds a valid PTA license and is open to therapists at all experience levels, with duties ranging from direct patient treatment to progress notes and discharge planning under supervision of a Physical Therapist.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders under the supervision of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides consultation and counseling to patients, families, caregivers and other service providers related to physical disorders.
• Supports discharge planning and summary of services provided as required.

$42 Hourly!

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
