Job summary

Continuum Rehab Group seeks a Licensed Physical Therapy Assistant to join our team providing in-house therapy with Skilled Nursing facilities across Texas, New Mexico, Indiana, Arizona, and Missouri.

Open to therapists at all experience levels, this on-site role focuses on delivering rehabilitative, skilled care under the supervision of a Physical Therapist, utilizing evidence-based practice and documenting progress for discharge planning and continuity of care.

Qualifications

• Licensed Physical Therapy Assistant credential.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under the supervision of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Contributes to discharge summaries of services provided.

This position is open to therapists at all experience levels.

Some Duties For This Position Include

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
