Job summary

Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to support in-house therapy across Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona, and Missouri.

Applicants must be licensed as a Physical Therapy Assistant and may be considered at all experience levels. Duties include evidence-based interventions, patient progress assessment, and collaboration with a supervising Physical Therapist.

Qualifications

• Licensed Physical Therapy Assistant with current licensure.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress and communicates with supervising Physical Therapist.
• Provides counseling to patients and families regarding physical disorders.
• Contributes to discharge planning and documentation as required.

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
