Job summary
Creative Solutions in Healthcare, Inc. in Denton, TX is seeking a compassionate Physical Therapy Assistant (PTA) on a PRN basis to join our in-house rehab team.
You will provide person-centered care to help residents improve mobility, strength, and quality of life under PT supervision. The role requires an active Texas PTA license, familiarity with SNF/long-term care settings, and experience with Net Health.
Qualifications
- Active and valid Texas Physical Therapy Assistant license required.
- Experience in Skilled Nursing Facility or long-term care preferred.
- Familiarity with Net Health or equivalent EMR systems is a plus.
Responsibilities
- Administer medically prescribed physical therapy treatments to relieve pain and improve function.
- Provide skilled PT services under the supervision of a Physical Therapist.
- Respond to service requests and relay information to the PT.
- Implement and modify care plans under PT supervision.
- Measure and record motor function, strength and performance.
- Monitor patient responses and adjust approaches with the interdisciplinary team.
- Provide education to patients, families, and caregivers.
- Ensure timely documentation in compliance with policies and regulations.
- Attend required rehab department meetings.
