Job summary

Peachtreenursing in Weatherford, Texas is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house Rehab team on a PRN basis.

You will administer medically prescribed PT, provide skilled services under a Physical Therapist, and monitor patient progress to improve mobility and quality of life.

The ideal candidate holds an active Texas PTA license, has SNF or long-term care experience, and can document accurately using Net Health in a collaborative, patient-centered environment.

Qualifications

• Active Texas PTA license required.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health is a plus.

Responsibilities

• Administer medically prescribed PT treatments to relieve pain and improve function.
• Provide skilled PT services under supervision of a Physical Therapist and per physician orders.
• Respond to service requests and relay information to the PT.
• Implement and modify care plans under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adjust approaches with the team.
• Provide education to patients, families, and caregivers.
• Ensure timely, compliant documentation per policies and regulations.
• Attend required rehab department meetings.
