Job summary

Creative Solutions in Healthcare - South is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house team on a PRN basis. You will provide person-centered care to help residents improve mobility and quality of life under supervision of a Physical Therapist.

Key requirements include an active Texas PTA license, good standing with regulatory boards, and knowledge of physical therapy practices. Net Health EDS experience is preferred.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• In good standing with all regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer medically prescribed physical therapy treatments under direction of a Physical Therapist.
• Provide skilled PT services and interventions under physician orders.
• Respond to service requests; relay information and referrals to the PT.
• Implement and modify patients' plans of care under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adjust approaches in collaboration with the team.
• Provide education to patients, families, and caregivers.
• Ensure timely and thorough documentation per policies and regulatory requirements.
• Attend required meetings as designated by Rehab Director.
