Job summary

Grace Pointe Wellness is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis in Texas. You will provide person-centered care to help residents improve mobility, strength and overall quality of life under the supervision of a Physical Therapist.

Active Texas PTA license and good standing with regulatory boards are required. SNF or long-term care experience is preferred, with knowledge of Net Health documentation.

Qualifications

• Active and valid Texas Physical Therapy Assistant license
• In good standing with regulatory agencies and licensing boards
• Skilled nursing facility (SNF) or long-term care experience preferred
• Current knowledge of physical therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication and teamwork skills

Responsibilities

• Administer prescribed physical therapy treatments under the supervision of a Physical Therapist
• Provide skilled PT services and interventions per physician orders
• Relay information and referrals to the Physical Therapist
• Implement and modify care plans under PT supervision
• Measure and record motor function, strength, and muscle performance
• Monitor patient response and adjust approaches with interdisciplinary team
• Provide clinical support and education to patients, families, and caregivers
• Ensure thorough documentation in compliance with policies and regulations
• Attend required meetings designated by the Director of Rehab
