Job summary

Lebanon Center for Rehabilitation and Healing seeks a Physical Therapist (PT) for our facility to provide PRN weekday coverage.

The PT will assess referred patients, design and manage treatment programs, document progress, and collaborate with the care team to maximize outcomes for geriatric patients.

This role involves supervising Physical Therapy Assistants, ensuring HIPAA compliance, and delivering compassionate, patient-centered care with reliable attendance.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Valid state license and registration.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term or geriatric care preferred.
• Proficient in clinical documentation.
• Familiar with PPS scheduling and Case Mix Index preferred.
• Excellent patient service and professional conduct.
• Punctual and reliable with scheduled hours.
• HIPAA confidentiality compliance.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals per physician guidance.
• Document patient status and progress; educate patients and families.
• Collaborate with all disciplines to deliver patient-centered care.
• Supervise Physical Therapy Assistants; ensure compliance and safety.
• Maintain confidentiality and adhere to HIPAA and residents' rights.
