Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for our facility. The PT will assess referred patients, develop treatment plans, and implement a program under physician direction to improve functional capacity.

This PRN weekday role requires a valid PT license, experience with geriatric care, and strong documentation. Collaboration with the care team and timely patient progress notes are essential.

Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Must hold a valid professional license and registration for the state of practice.
• Experience supervising Physical Therapy Assistants is required/preferred.
• Experience with long-term care and/or geriatric care is preferred.
• Strong documentation skills and accurate record-keeping.
• Experience with evaluations, progress notes, recertification and discharge documentation.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Develop and administer treatment plans and goals per physician direction.
• Document patient conditions and educate patients and families.
• Collaborate with other disciplines to provide patient-centered care.
• Maintain professional standards and timely attendance.
