Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) to join our facility on a PRN Weekdays schedule. The PT will assess referred patients, develop and implement treatment plans, and work under the direction of the Director of Rehabilitation.

Licensure and certification must be current, and experience with geriatric care is preferred. This role emphasizes collaboration with an interdisciplinary team, accurate documentation, and exceptional patient service as part of a

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed and registered to practice in the state.
• Experience supervising assistants.
• Experience in long-term or geriatric care preferred.
• Skilled in clinical documentation.
• Familiar with case mix index and PPS scheduling preferred.

Responsibilities

• Assess referred patients and provide direct patient care.
• Establish and administer treatment plans with goals.
• Document patient condition and progress.
• Educate patients and families on therapy methods.
• Collaborate with other disciplines to ensure patient-centered care.
• Maintain confidentiality and comply with HIPAA.
