Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) to assess patients, develop treatment programs, and document progress under physician direction. Responsibilities include supervising PT assistants, collaborating with a multidisciplinary team, and ensuring high-quality patient care.

Candidates must be licensed, with graduation from an accredited PT program and experience in geriatric care preferred.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment.
• Exceptional patient service skills and professionalism.
• HIPAA confidentiality and Residents Rights compliance.

Responsabilités

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer individualized treatment programs with goals.
• Document patient condition and educate patients and families.
• Supervise Physical Therapy Assistants and coordinate with the Director of Rehabilitation.
