Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for our facility. The therapist will assess patients requiring rehabilitation, develop and manage treatment programs, and document progress under physician direction.

Responsibilities include supervising PTAs, ensuring timely progress notes, and coordinating with other disciplines to deliver patient-centered care. PRN weekday availability is required.

Formación

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Ability to write clear documentation including evaluations and progress notes.
• Maintain PPS scheduling and case mix awareness where applicable.

Responsabilidades

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with specific goals.
• Document patient condition and educate patients and families in physical therapy methods.
• Collaborate with all disciplines to provide patient-centered care.
• Report to work on time and maintain professionalism at all times.
• Perform duties as assigned by the Director of Rehabilitation.
