Lebanon Health and Rehabilitation Center seeks a licensed Physical Therapist (PT) for PRN weekday coverage. Provide direct patient care, assess functional status, and coordinate with physicians under rehabilitation direction at a long-term care facility.

The role includes supervising Physical Therapy Assistants, documenting progress, and maintaining confidentiality while delivering patient-centered care to geriatric residents.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Must hold a valid PT license/registration for the state of practice.
• Experience with long-term care or geriatric patients preferred.
• Strong documentation and note-writing skills.
• Ability to collaborate with other disciplines to deliver patient-centered care.
• Punctual and professional with reliable scheduling.

Responsabilités

• Assess referred patients and develop individualized therapy plans.
• Provide direct patient care and document progress notes.
• Oversee and supervise Physical Therapy Assistants.
• Adhere to confidentiality, HIPAA, and residents' rights.
• Report to Director of Rehabilitation as required.
• Participate in PPS scheduling and Case Mix considerations when applicable.
