Lebanon Health and Rehabilitation Center seeks a Physical Therapist (PT) for PRN weekday coverage. You will assess patients, design treatment plans, and document progress under physician direction. Supervision of PTAs and collaboration with a multidisciplinary team are essential.

Experience in long-term or geriatric care is favored. You must maintain HIPAA compliance and provide high-quality patient-centered care in a nursing facility setting.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid license for the state of practice.
• Experience with long-term care/geriatric care preferred but not required.

Responsabilités

• Assess referred patients and develop treatment plans.
• Provide direct patient care and set realistic goals.
• Supervise Physical Therapy Assistants and collaborate with disciplines.
• Document progress notes, discharge summaries, and PPS scheduling as needed.
