Job summary

Cedar Healthcare Center is seeking a dedicated Physical Therapist to assess, plan, and implement individualized therapy programs to maximize functional progress. You will document outcomes, collaborate with physicians and staff, and educate patients and families on care plans.

Responsibilities include direct patient care, care-plan design and adjustments, adherence to standards of care, and participation in ongoing professional development to improve patient outcomes in our facility.

Qualifications

• Graduate of an accredited program (Bachelors, Masters, or Doctorate) in Physical Therapy.
• Current license from the state Board of Physical Therapy Examiners.
• Proficiency with applicable state, federal, and professional standards.

Responsibilities

• Evaluates patient needs and outcomes, and adjusts care plans accordingly.
• Provides direct patient care and designs plans of care based on assessments.
• Documents evaluations, treatment goals, and plans; maintains records.
• Communicates effectively with other professionals and support staff to ensure positive patient outcomes.
• Adheres to facility policies, safety, and infection control standards.
• Participates in ongoing professional development and staff education.
