The role requires PA licensure, graduation from an accredited PT program, and a commitment to compassionate, resident-centered care in a skilled nursing/rehabilitation setting.

Qualifikationen

• Must hold a PA Physical Therapist license and be licensed to practice in Pennsylvania.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification or ability to obtain.

Aufgaben

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services based on resident needs and goals.
• Implement therapeutic exercises, mobility training, balance activities, transfers, and functional training.
• Monitor progress and modify treatment plans as appropriate.
• Maintain accurate and timely clinical documentation.
• Communicate with residents, families, physicians, nursing staff and care team.
• Participate in interdisciplinary care planning and team meetings.
• Promote safety, mobility, independence and quality of life.
• Maintain compliance with state and facility policies.
• Provide resident-centered care with professionalism and compassion.
