Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a skilled nursing facility setting. You will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff to ensure excellent care, set measurable goals, educate patients and families about progress and outcomes, supervise Physical Therapist Assistants, and complete timely documentation throughout the patient care journey.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes
