Job summary

Willow Tree seeks a compassionate Physical Therapist to join our skilled nursing team in Bryan, Ohio. The role involves delivering rehabilitative therapy to residents, documenting outcomes, and collaborating with a multidisciplinary team to restore function and enhance quality of life.

The position is PRN with pay rate dependent on experience. Candidates should be licensed to practice in Ohio and have 2+ years in SNF settings, with strong communication and documentation skills to support patient

Qualifications

Bachelor's degree in Physical Therapy or related field is required.
Active state license is required.
2+ years of experience in a skilled nursing facility environment.

Responsibilities

Screen and evaluate patients with cognitive/ADL physical functioning disorders and develop a care plan.
Use standardized assessments and evidence-based practices to guide interventions.
Provide rehabilitative, skilled, and medically necessary treatment per standards.
Perform ADL retraining, therapeutic activities, exercises, modalities, and assistive device use.
Monitor patient progress and adjust treatments in the clinical record.
