Willow Tree Post Acute is seeking a Physical Therapist to provide rehabilitative therapy in a skilled nursing setting. You will evaluatet patients, develop plans of care, and deliver evidence-based interventions to improve function and ADLs.

The role requires an active state license, a Bachelor’s degree in Physical Therapy, and 2+ years in a SNF environment. You will collaborate with the rehabilitation team and document outcomes and discharge summaries.

Qualifications

• Bachelor's degree or higher in Physical Therapy or related field.
• Active state license.
• 2+ years of experience in a skilled nursing facility environment.

Responsabilités

• Screen and evaluate patients with cognitive and/or ADL disorders and develop a care plan.
• Provide rehabilitative, skilled and medically necessary treatment.
• Administer ADL retraining, therapeutic activities and exercises.
• Document progress and adjust treatments in the clinical record.
• Communicate with patients, families and health team members about progress.
• Provide discharge summaries per regulatory requirements.
