Job summary

Willow Tree in Ohio is seeking a dedicated Physical Therapist to provide rehabilitative therapy in a skilled nursing facility. You will evaluate patients, create care plans, and work with a multidisciplinary team to improve function and quality of life for residents.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop a plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatments per regulatory standards.
• Document treatments and progress in the electronic medical records system.
• Communicate progress and plan with supervising therapist and health care team.
• Supervise and support physical therapist assistants and aides as needed.
