Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Paterson, NJ. This PRN role focuses on delivering high-quality physical therapy services to residents, while adhering to CMS documentation and billing requirements.

Qualified candidates hold a BS in Physical Therapy, an active license in the applicable state, and commit to ongoing education to maintain licensure.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct regular screenings to determine intervention needs.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and plans to residents, families, and the interdisciplinary team.
• Complete compliant, medically necessary documentation per CMS expectations in NetHealth EMR.
• Perform other duties as assigned to support resident care.
