Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist to provide in-house therapy services at a Skilled Nursing Facility in Lawrence Township, NJ. The role is PRN and focuses on planning, implementing, and documenting therapy interventions in collaboration with the interdisciplinary team.

The ideal candidate will hold a BS in Physical Therapy and an active PT license, with commitment to ongoing professional development and high-quality resident care in a regulated healthcare setting.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to develop treatment plans with short- and long-term goals.
• Conduct regular screenings to determine need for intervention/treatment.
• Develop treatment plans and communicate findings to residents and families.
• Complete compliant CMS-dictated documentation in NetHealth EMR systems.
