Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist to join our partner facility's in-house Skilled Nursing Facility in Somerset, PA. This PRN role focuses on delivering high-quality PT services in accordance with established practice and CMS guidelines.

You will perform screenings, evaluations, and treatment planning, while ensuring accurate documentation in NetHealth EMR and collaboration with the interdisciplinary team for optimal resident care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and department procedures.
• Adhere to documentation and billing procedures.
• Conduct screenings and evaluations to determine intervention needs.
• Develop treatment plans with short and long-term goals.
• Communicate results and recommendations to residents and interdisciplinary team.
