Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Lawrenceville, NJ. This PRN role focuses on planning, delivering, and directing PT services to residents while ensuring compliance with current standards and CMS requirements.

You will evaluate residents, develop treatment plans, communicate progress to families and the interdisciplinary team, and maintain thorough documentation in NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Demonstrate sound clinical judgment in evaluating and planning resident therapy.
• Screen residents to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning.
• Develop and implement treatment plans with short and long-term goals.
• Communicate findings and plans to residents, families, and the care team.
• Complete compliant documentation in CMS-compliant EMR systems.
