Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist to join our in-house Skilled Nursing Facility program in Johnstown, PA. This PRN role focuses on delivering quality resident care and collaborating with an interdisciplinary team.

Responsibilities include evaluating residents, developing treatment plans, documenting per CMS standards in NetHealth EMR, and ensuring compliance with standards and regulations. Active PT license required and ongoing CE participation encouraged.

Qualifications

• Must have a Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures.
• Develop treatment plans with short and long-term goals and communicate findings to residents and team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Interact with residents, families, and interdisciplinary team members.
