Qualifications

• Qualified Physical Therapist with state license or eligible.
• Experience in SNF or long-term care settings preferred.

Responsibilities

• Provide evaluation and treatment to patients following an individualized plan of care.
• Deliver excellent care and customer service to patients and families.
• Collaborate with facility staff to ensure high-quality patient care.
• Evaluate new patients, develop plans of care, and set measurable goals.
• Treat patients, address concerns, and educate on progress and outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate patient progress and adjust plans of care as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
