Job summary

Enhance Rehabilitation in Illinois is seeking qualified Physical Therapists to join our SNF team. As a Physical Therapist, you will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to deliver high-quality outcomes.

You will supervise Physical Therapist Assistants, document evaluations and progress notes, and communicate with patients and families about progress and expected outcomes.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
