Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a skilled nursing facility setting. The role involves evaluating and treating patients according to individualized plans of care and collaborating with facility staff to ensure high-quality outcomes.

You will supervise Physical Therapist Assistants, document evaluations and progress, and communicate effectively with patients and families about injury, progress, and expected outcomes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing concerns and educating them on injury, progress and outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Collaborating with facility staff to ensure excellent patient care
