Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a Skilled Nursing Facility setting. You will evaluate and treat patients following an individualized plan of care and work closely with facility staff to maintain high-quality care.

The role includes supervising Physical Therapist Assistants, updating progress notes, and communicating effectively with patients and families about injury, progress, outcomes, and discharge plans.

• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
