Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in SNF settings. You will evaluate and treat patients following individualized plans of care, with a focus on measurable progress and functional outcomes.

You will collaborate with facility staff, educate patients and families about injury, progress, and expected outcomes, and supervise Physical Therapist Assistants while ensuring timely documentation and quality care.

Responsibilities

• Evaluate and treat patients following an individualized plan of care
• Provide excellent care for SNF patients
• Provide excellent customer service to patients and family members
• Collaborate with facility staff to ensure excellent patient care
• Evaluate new patients, develop plans of care and set measurable, functional goals
• Treat patients, answer concerns and educate them on injury, progress and outcomes
• Supervise Physical Therapist Assistants
• Re-evaluate patient progress, revise plans of care and reset goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
