Job summary

Enhance Rehabilitation in Crestwood, IL is seeking licensed Physical Therapists to join our growing SNF team. You will evaluate and treat patients according to individualized plans of care, help set functional goals, and collaborate with facility staff to deliver high-quality care daily.

You will supervise Physical Therapist Assistants, document evaluations and progress notes timely, and educate patients and families about injury and recovery.

Qualifications

• Licensed PT in the United States with current credentials.
• Experience in SNF or long-term care preferred.
• Ability to develop measurable functional goals.

Responsibilities

• Evaluate and treat patients per individualized plan of care.
• Provide excellent care and customer service to patients and families.
• Collaborate with facility staff to ensure high-quality care.
• Supervise Physical Therapist Assistants.
• Complete timely documentation including evaluations, re-evaluations, discharges and progress notes.

In this role, you will evaluate and treat patients following an individualized plan of care.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
