Job summary

CommuniCare Family of Companies is seeking a Physical Therapist to evaluate patients, develop and implement treatment plans, and supervise PTAs across multiple centers. The role involves leading interdisciplinary meetings, sharing clinical findings, and providing staff and family education as appropriate.

The position focuses on delivering patient-centered care within a network of facilities, promoting continuity of care and high-quality outcomes.

Qualifications

• Bachelor’s/Master’s/Doctorate Degree in Physical Therapy from an Accredited Program is required.
• Valid or pending State Physical Therapy License is required.

Responsibilities

• Evaluate, review, and screen patient charts and evaluations under physician order and develop a treatment plan.
• Implement treatment plan and provide supervision for PTAs.
• Lead interdisciplinary meetings to relay findings and make recommendations to rehab team and provide staff/family training when appropriate.

Job Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
