Lebanon Health and Rehabilitation Center in Smyrna, TN is seeking a licensed Physical Therapist for PRN weekday coverage. The PT will assess patients, develop and oversee rehab plans, and document progress under physician direction.

The role requires strong documentation skills, experience with geriatric populations, and collaboration with a multidisciplinary team to deliver patient-centered care. Competitive compensation and flexible weekday hours offered.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and state registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term or geriatric care preferred.
• Proficient in clinical documentation and record-keeping.
• Ability to conduct evaluations, progress notes, discharge planning.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrates confidentiality, HIPAA compliance and professionalism.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsabilités

• Assess referred patients and provide direct care.
• Establish and administer treatment programs with goals.
• Document patient condition and progress weekly.
• Educate patients and families on therapy methods.
• Oversee PT Assistants and collaborate with other disciplines.
• Maintain HIPAA confidentiality and professional behavior.
• Perform additional duties as assigned by Director of Rehabilitation.
