Job summary

Lebanon Health and Rehabilitation Center in Walterhill, TN, seeks a Physical Therapist (PT) for PRN weekday coverage to assess referred patients and develop individualized rehab plans under physician direction.

The role includes direct patient care, documentation of progress, and supervision of Physical Therapy Assistants, with a focus on interdisciplinary collaboration and excellent patient service.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed to practice as a Physical Therapist in the state.
• Experience supervising Physical Therapy Assistants preferred.
• Familiarity with long-term care/geriatric settings a plus.
• Strong documentation and record-keeping skills.
• Experience in evaluations, progress notes, recertifications and discharges.
• Willingness to collaborate with all disciplines for patient-centered care.
• Commitment to patient service, clinical excellence, and professionalism.
• Punctual, reliable, and adheres to schedule.
• Performs duties assigned by Rehabilitation Director.
• Confidentiality and HIPAA compliance mandatory.
• Experience with PPS scheduling is a plus.

Responsibilities

• Assess referred patients and develop rehab plans and goals.
• Provide direct patient care and implement treatment under physician direction.
• Document patient status, progress, and discharge planning.
• Educate patients and families on therapy methods.
• Supervise and oversee Physical Therapy Assistants.
• Collaborate with other disciplines to optimize patient outcomes.
• Maintain confidentiality and professional standards.
• Adhere to scheduled hours and professional conduct.
• Perform other duties as assigned by the Director of Rehabilitation.
