Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for PRN weekday coverage at our facility. The PT will assess referred patients, develop individualized treatment plans, and document progress while coordinating with the care team.

The role requires licensure, graduation from an accredited PT program, and experience in long-term or geriatric care is preferred. Strong documentation and patient-centered service are essential components of the position.

Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Must hold a valid professional license and registration for the state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and geriatric care is preferred.
• Strong documentation skills and ability to write evaluations and progress notes.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer individualized treatment programs based on patient capacity and tolerance.
• Document patient conditions and educate patients and families in therapy methods.
• Coordinate with interdisciplinary team to ensure patient-centered care.
• Report to work on time and maintain professional standards.
