Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) to work PRN weekdays at our facility. You will assess referred patients, develop treatment plans, and supervise PTAs, delivering high-quality, patient-centered care.

This role requires a degree in physical therapy, a valid license, and strong documentation. Experience in long-term or geriatric care is preferred; commitment to confidentiality and teamwork is essential.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Oversee and supervise Physical Therapy Assistants.
• Strong documentation writing and record-keeping.
• HIPAA/confidentiality awareness.
• Collaborate with multiple disciplines for patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Develop and administer individualized treatment plans.
• Document patient condition and progress; update records.
• Educate patients and families on therapy methods.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with all disciplines to ensure patient-centered care.
• Maintain confidentiality and professionalism.
• Adhere to scheduled hours and institutional policies.
