Job summary

Johns Island Post Acute is seeking a compassionate Physical Therapist for a PRN role in Johns Island, SC. You will evaluate residents, develop treatment plans, and supervise PT assistants while coordinating with the care team to improve mobility and independence.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• SC Physical Therapist license or eligibility is required.
• Masters/Doctorate preferred.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and educate staff as needed.
• Maintain timely, accurate documentation per policies and payer requirements.
• Collaborate with physicians, families, and interdisciplinary team.
