Job summary

Gardens of Belden Village is seeking a compassionate and motivated PRN Physical Therapist to join our rehabilitation team. The PT will evaluate residents, develop treatment plans, and deliver interventions to improve mobility and independence.

The role involves collaboration with nursing, physicians, residents, and families, with opportunities to educate and discharge-plan. Ohio license and accreditation are required; CPR is preferred.

Qualifications

• Graduate of an accredited Physical Therapy program required.
• Ohio Physical Therapist license in good standing required.
• CPR certification preferred.
• Experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred.
• Experience with Net Health documentation software preferred.
• Experience with PCC preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on findings and goals.
• Provide skilled PT interventions to improve mobility, strength, balance, and independence.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, treatments, progress notes, and other clinical information.
• Collaborate with interdisciplinary team to support care goals and discharge planning.
• Educate residents, families, and caregivers on plans, safety, and functional strategies.
• Maintain compliance with policies, standards, and regulations.
• Participate in care conferences and team meetings.
• Maintain a safe and therapeutic treatment environment.
