Responsibilities

• Evaluate, plan, and administer treatment to residents to relieve pain and restore function.
• Deliver treatment per care plan and modify as needed.
• Provide clinical guidance to patients, families, and caregivers.
• Record motor function, strength, and muscle performance.
• Monitor responses and adjust treatments accordingly.
• Complete documentation per policy and state/federal regulations.

We are seeking a PRN Physical Therapist to join our amazing in-house rehab team at Kadima Rehabilitation & Nursing at Washington

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
